21 March 2014 marks the 9th anniversary of World Down Syndrome Day and each year the voice of people with Down syndrome, and those who live and work with them, grows louder. But there is still so much more we can do.



all people with Down syndrome have the right to access healthcare when required on an equal basis with others without discrimination and with proper assessment of the specific health needs of the individual.  And there are important points everyone should know:
  1. Having Down syndrome does not make a person unhealthy.
  2. Down syndrome is a genetic condition, not an illness.
  3. People with Down syndrome may have health issues throughout their lives, just like everyone else and they should have access to healthcare on an equal basis with others.
  4. There are specific known health issues which may affect people with Down syndrome, for which accurate, evidence based information is available.
  5. Health professionals should be aware of these specific issues when treating a person with Down syndrome.
  6. Health professionals should not discriminate against people with Down syndrome by:
  • refusing to treat them;
  • blaming health issues on Down syndrome in general, or;
  • considering only specific known health issues which may affect people with Down syndrome

21 مارس 2014 يصادف الذكرى السنوية 9 ليوم متلازمة داون  العالمي كل عام صوت الناس الذين يعانون من متلازمة داون ، وأولئك الذين يعيشون ويعملون معهم ، وينمو عاليا . ولكن لا يزال هناك أكثر من ذلك بكثير مما يمكننا القيام به.

جميع الناس الذين يعانون من متلازمة داون لديهم الحق في الحصول على الرعاية الصحية عند الحاجة  مثل  الآخرين دون تمييز و تقديم التقييم السليم للاحتياجات الصحية الخاصة للفرد.  و هنالك نقاط مهم يجب على الجميع معرفتها :

  1. وجود متلازمة داون لا تجعل الشخص غير صحي.
  2. متلازمة داون هي حالة وراثية ، وليس المرض.
  3. الأشخاص الذين يعانون من متلازمة داون قد يكون لديك مشاكل صحية طوال حياتهم ، تماما مثل أي شخص آخر، و أنها ينبغي أن يكون الحصول على الرعاية الصحية على قدم المساواة مع الآخرين.
  4. هناك قضايا صحية معروفة محددة والتي قد تؤثر على الناس الذين يعانون من متلازمة داون ، وهنالك  معلومات دقيقة و تستند إلى الأدلة مبنيه على دلائل علميه.
  5. يجب أن يكون الاخصائين الصحيين و المعلمين  على علم  بالمشاكل  التي  يعانونها من متلازمة داون  .
  6. يجب أعلى الاخصائيين  الصحيين  و المعلمين ان لا يميزوا و يكونوا  ضد الأشخاص ذوي متلازمة داون من قبل :
  • رفض التعامل معهم ؛
  • إلقاء اللوم على القضايا الصحية على متلازمة داون بشكل عام ، أو ؛
  • النظر في القضايا الصحية المعروفة المعينة فقط والتي قد تؤثر على الناس الذين يعانون من متلازمة داون

Auditory Integration Training and Autism- علاج التكامل السمعي لآطفال التوحد


Auditory integration training – also known as AIT – involves a person listening to a selection of music which has been electronically modified.
There are several different kinds of auditory integration training including the Berard Method, the Listening Program, Samonas Sound Therapy, and the Tomatis Method
AIT is based on the idea that some people, including some people with autism, are hypersensitive (over-sensitive) or hyposensitive (under-sensitive) to certain frequencies of sound.
This sensitivity to certain frequencies is believed to cause a variety of perceptual problems – such as an inability to concentrate or to understand other people. It may also cause other problems – such as irritability or lethargy.
AIT is designed to improve the person’s ability to process sounds by ’re-educating’ the brain. This is done by playing electronically modified music in which the frequencies have been changed.

Research Opinion

There is some high quality research evidence to suggest that AIT is not helpful in improving symptoms of autism, although we believe it may be of limited use in helping with sensory problems, such as hypercusis.
Great care should be taken because of the potential hazards of using AIT.
Opinion on the usefulness of AIT is mixed but the research evidence is generally not supportive.
  • There is no evidence to support claims of cure or recovery.
  • There is very poor evidence that AIT has been helpful in improving symptoms of autism.
  • There is mixed evidence on whether AIT has been helpful in improving the behaviour and functioning of individuals with autism and other sensory problems, such as hypercusis.
There are numerous anecdotal reports in support of AIT.
It should also be noted that the relationship between hypercusis and autism is not clear.

- العلاج بالتكامل السمعي ، هو عبارة عن الاستماع لاصوات معالجة بالكمبيوتر لإزالة الأصوات التي تسبب الحساسية المفرطة والتقليل من حدة الأنماط الصوتية .
- يستعمل جهاز خاص لمعالجة أو تعديل الاصوات لجلسة العلاج ، يوجد بعض الجدل في مدى تأثير أو فعالية العلاج بالتكامل السمعي للتقليل من الحساسية المفرطة تجاه الأصوات .
تدريب التكامل السمعي تم تطويره بواسطة دكتور/جاي بيرارد, وهو اختصاصي ممارس في طب الأنف والأذن والحنجرة (طبيب انف وأذن وحنجرة)في مدينة انسي)في فرنسا.الدكتور بيرارد هو في الأصل مخترع تدريب التكامل السمعي لإعادة تأهيل اضطرابات الجهاز السمعي مثل فقدان السمع أو تشوه السمع(السمع فائق الحدة أو الغير متماثل)بعد(30)سنة من الممارسة الإكلينيكية والدراسة ,حدد الدكتور/بيرارد التشوهات في السمع او المعالجة لسمعية التي في الغالب تساهم في العديد من الاضطرابات السلوكية أو اضطرابات التعلم. وهو قد استخدم بنجاح منهج تدريب التكامل السمعي لمساعدة الأشخاص الذين يعانون من اضطراب نقص الانتباه المفرط النشاط والمشاكل الحسية والتوحد وعسر القراءة وفرط النشاط وعدم القدرة على التعلم والقصور اللغوي واضطرابات النمو الواسعة الانتشار وخلل المعالجة السمعية المركزية وخلل نقص الانتباه والاكتئاب.في اغلب الحالات التي اشرف عليها الدكتور/بيرارد تدريب التكامل السمعي قلل بشكل واضح بعض الإعاقات المرتبطة مع الاضطرابات المذكورة أعلاه مبادئ تدريب التكامل السمعي وملخص تواريخ حالات مختلفة مع نسخ كاملة لاختبار السمع الفردية قبل وبعد تدريب التكامل السمعي,ممثلة في كتاب الدكتور/بيرارد:السمع يساوي السلوك,والذي تم نشره في فرنسا في سنة1982م النسخة المنقحة تم نشرها باللغة الانجليزية في سنة 1993م بواسطة "شركة كيتس للنشر" .

 رأي  لابحاث العلميه  
هناك بعض الأدلة البحثية ذات جودة عالية لتوحي بأن علاج التكامل السمعي ليس مفيد في تحسين أعراض مرض التوحد، على الرغم من انه يعتقد أنه قد تكون ذات فائدة محدودة في مساعدة الذين يعانون من مشاكل حسية

 يجب توخي الحذر الشديد بسبب المخاطر المحتملة لاستخدام علاج التكامل  السمعي 

رأي حول جدوى التكامل السمعي  مختلطة ولكن الأدلة البحثية عموما ليست داعمة. 

  •  اليس هناك أي دليل لدعم المطالبات من علاج أو الشفاء التوحد . 
  • هناك أدلة الفقيرة جدا التي تثبت الفائده  في تحسين أعراض مرض التوحد. 
  • هناك أدلة متضاربة بشأن ما إذا كان التكامل السمعي مفيد في تحسين سلوك وأداء الأفراد الذين يعانون من التوحد والمشاكل الحسية الأخرى، 

وهناك العديد من الاقوال  لدعم  التكامل السمعي 

كما تجدر الإشارة إلى أن العلاقة بين مرض التوحد واحتداد السمع غير واضح.

طرق التحفيز اللغوي للطفل




تعلم اللغة عن طريق  ممارستها في  الحياة اليوميه . ومع ذلك، فإن السنوات الخمس الأولى من الحياة هي الأكثر أهمية  لإقامة قاعدة لغوية قوية . تعتمد على ممارسات التحفيز اللغة مما يجعل عددا من التغييرات الطفيفة في تعاملك وسلوكياتك مع طفلك ، والتي قد تعمل على تسهيل تطوير اللغة

التحفيز اللغة عن طريق غير المباشرة  وهي عبارة عن مجموعة من الطرق لتحفيز اللغة. و الهدف الاساسي  للكبار ، في استخدام هذه الطرق هو مساعدة الأطفال الصغار على التعلم والتحدث.


  • لا تقوم بتلبية طلبات الطفل مباشره 

  •  انتظر  . تأخير ردودكم على تأشير طفلك ، والإيماءات أو الحركات عندما يريد الأشياء. تظاهر كآنك  لا تفهم ما يريد  سيحاول طفلك  التواصل لفظيا . التوقف و اتاحة  الفرصة لطفلك  للتعبير عن نفسه لفظيا .

  • اخلق الفرص لاستخدام اللغة : هل يحتاج طفلك استخدام الكلمات من أجل التواصل ؟ العديد من الأسئلة التي تسأل قد تتطلب أطفالنا بالاجابه فقط بنعم او لا . لزيادة الحاجة إلى التواصل لفظيا ، قم بمحاولة طرح السؤال الذي يتطلب استجابة لفظية (أي " ماذا تريد أن تشرب ؟ " ) .


  •  القراءة لطفلك :  أفضل شيء يمكنك القيام به للطفل هي قراءة الكتب معهم. اختيار الكتب الملونة مع صور  كبير،  بسيطة . تحدث عن الصور بدلا من مجرد قراءة النص . لتشجيع التفاعل مع طفلك قم بطرح الاسئلة عليه و  أشر إلى ما كنت تتحدث عنه

  • التحدث بلسان طفلك : يقوم الكبار بجمل قصيرة بوصف ما يقوم به الطفل ، وما يراه ،و ما يسمعه  ، على سبيل المثال، " أنا اكل تفاحه " ، "ارمي الكره ". استخدام الجمل قصيرة وبسيطة لمساعدة الطفل لمعرفة أن هناك كلمات لوصف الأشياء و الافعال.

  •  التكرار : يقوم الكبار بتكرار  بالضبط ما يقوله الطفل ، ولكن بإستخدام التعبير الصحيح. على سبيل المثال، يقول الطفل ، فإن " سا ام  فاح " ، و الكبار يقولون، " ساره اكلت تفاحه" .

  • تطويل الجمله. إضافة بضع كلمات و / أو معلومات إلى الكلام الطفل. على سبيل المثال ، الطفل : " محمد ذهب " الرئيسي / معلم : " محمد ذهب للمدرسه. " "

  •  المراجعة. الاستجابه للطفل بجمل صحيحه نحويا ، حيث المعنى واضح للمستمع . على سبيل المثال ، الطفل : "بابا حلاوه اشترى". المعلم / الوالد : "بابا اشترى لي حلاوه " .

  • تقديم الخيارات  بين  الجمله الغير صحيحة مقابل الجمله الصحيح. على سبيل المثال ، الطفل : " كان له ذلك مضحكا ! " المعلم / الوالد : " هل "، وكان له ذلك مضحكا 'أو' وكان ذلك مضحكا ؟ "

  •  تسمية الأشياء و الافعال لبناء مفردات الطفل. لا يحتاج طفلك إلى تكرار كلماتك، على الرغم من أنك يمكن أن تشجع هذا إذا كان الفعل  من تلقاء نفسه  ( ويكفي أن يسمعون التسميات وربطها الكائنات الصحيح أو الافعال الصحيحه ). مع طفل المتحدث يمكن تسمية صفات الاشياء ( على سبيل المثال حجم والشكل واللون ، وعدد و نسيج) ، والكلمات التي تصف كيف يتم تنفيذ هذا العمل (على سبيل المثال بسرعة ، لحسن الحظ ، بسهولة، بصوت عال ) أو المكان (مثل في / خارج ، على / قبالة ، تحت، وراء، بين ) .


  •  الغناء لطفلك . الغناء للطفل يساعد على جذب انتباه الطفل ، والاهتمامه ، والاستماع ، والكلام. غني الأغاني البسيطة لطفلك ليمكنه أن يغنيها أيضا. 

  • تذكر/ي في استخدام كل هذه الطرق التي تحتاج إلى أن تكون في نفس مستوى  طفلك لتسهيل التفاعل. إذا كان طفلك يتمدد على الأرض، تمدد في الارض بجانبه، إذا كان طفلك يجلس على طاولة صغيرة ، يجب أن تجلس على الأرض أو تجلوس على كرسي صغير بحيث كنت على مستوى عين طفلك.

LANGUAGE STIMULATION TECHNIQUES





Language learning is a life long pursuit.However, the first five years of life are the most important ones in which to establish a strong linguistic base to build upon.Language stimulation practices involve making a number of subtle changes in your interactions and behaviors with your child, which may serve to facilitate language development

Indirect language stimulation is a set of language enrichment techniques. The primary 
goal, for adults, in using these techniques is to help young children learn to talk. 


  • not anticipating need or desire 



  • Wait, wait, wait. Delay your responses to your child’s pointing, gestures or babbling when he wants things. Pretend you don’t understand what he wants to see if your child will try to verbally communicate. Pausing allows your child another chance to verbally express himself.


  • CREATE OPPORTUNITIES TO USE LANGUAGE: Does you child need to use words in order to communicate? Many of the questions we ask our children may require only a simple yes/no are pointing response. To increase the need to verbally communicate, try asking a question that requires a verbal response (i.e. “What would you like to drink?”). To increase the likelihood of a verbal answer, model a target response (i.e. “You’re thirsty. Would you like a cup of Juice or water?”). Encourage, rather than demand imitation.



  •  Read, Read, Read! The best thing you can do for a child is to read books with them. Choose colorful books with large, simple pictures. Talk about the pictures rather than just reading the text. Encourage interaction from your child by asking him/her questions. Point to what you are talking about



  • Parallel Talk (child centered) Short phrase (4­5 words) the adult says that describes what the child is doing, seeing, hearing, as he does it e.g., “You’re pushing the car”, “You are digging in the sand”, “You see the red bucket”. Give the child words to describe the action he is involved in.


  • Self­Talk The adult uses short sentences to describe what he is doing, seeing, hearing, e.g., “I am washing the cups”, “I see the yellow shovel”. Use short, simple sentences to help the child know there are words to describe things people do.


  • Open­ ended Questions Questions that are broad in their context; that allow for multiple responses and that do not limit the child to single word responses such as “it’s blue”, “yes” or  “no” responses. Good example of open­ ended question: “What do you think will happen if ….?,  or “I wonder what you could use this for?”. 


  • Repetition  Adult repeats exactly what the child says, but uses correct articulation. For example, the child says, “Widdle wed wabbit”, and the adult would say, “Little red rabbit”. 


  • Expansion. Add a few more words and/or information to the child’s utterance. e.g., Child: “Bus leaving.” Parent/Teacher: “The bus is leaving.”   Child: “We saw police.” Parent/Teacher: “Yes, we saw a policeman who taught us about safety.”  


  •  Revision. When responding to the child, provide a more organized utterance, where the meaning is clear to the listener. e.g., Child: “The two guys had money and otherwise he didn’t have money.” Teacher/Parent: “It’s a good thing his friends had money because he didn’t have any. Otherwise he would not have been able to buy his lunch.” 


  •  Offering Choices between the incorrect versus the correct structure. e.g., Child: “Him was so funny!” Teacher/Parent: “Is it, ‘Him was so funny’ or ‘He was so funny’?” 


  • Labelling Label common objects and actions to build vocabulary. Your child does not need to repeat your words, although you can encourage this if they do it spontaneously (it is sufficient that they hear the labels and connect them to the correct objects or actions - comprehension comes before expression). With a more verbal child you can label qualities of objects (eg. size, shape, colour, number, texture), words describing how an action is performed (eg. quickly, happily, easily, loudly) or place words (eg. in/out, on/off, under, behind, between).





  •  Sing to Your Child. Song promotes vocal play, attention, listening, and speech. Sing simple songs your child can sing too. (Ex. The Itsy Bitsy Spider; The Little White Duck; Twinkle, Twinkle, Wheels on the Bus).



REMEMBER in using all of these techniques that you need to be at the same physical level as your child to  facilitate interaction. If your child is lying on the floor, you should be lying on the floor too, if your child is sitting  at a small table, you should kneel on the floor or sit in a small chair so that you are at your child's eye level.

  I think all the details make everything clear. The author presents the material in a comprehensive manner.  It offered different ways of writing professional reports and progress notes.  It is an excellent reference book that every beginning clinician absolutely must have.
